Environmental Health and Safety, with the support of Risk Management and Medical Leaves Administration, supports the University Injury/Illness/Exposure Reporting process.
The on-line reporting system allows staff, faculty, students, and guests to report injuries, illnesses, or potential exposures to the University. This system does not contact emergency services - please call 607-255-1111 for emergencies.
This report must be completed for all physical injuries, work-related illnesses, and potential hazardous substance exposures. Employees must report these incidents to their supervisor immediately.
Please complete all sections of this form when you have collected the necessary information to accurately answer all questions, including:
- Detailed incident description
- Medical treatment provided
- Lost work day status (Y/N)
- Corrective actions taken

Cornell University facilities are required to have a suitable First Aid kit specific to their work environment. The information below is meant to help guide supervisors and facility managers in the selection of a suitable first aid kit.
